The --code-emphasis flag repairs emphasis markers adjoining inline code
spans. Inside buffered table cells, the repair runs before column widths are
measured, so the reflow uses the final cell text and produces aligned output in
the same pass.
For non-table content, --code-emphasis retains its existing behaviour and
repairs the same inline-code and emphasis-marker combinations as before.
